Allows store administrators to dynamically add as many standard text box fields to their "Contact" form as they need, without assitance or programming. The information collected will be emailed to the store admin email account with the data in distinct fields:

• you can make the id labels (the filed names themselves are automatically system generated)
• the additional information will be included in the subsequent email sent
